Parents may access all required enrollment paperwork by clicking the links below.

Please complete the forms on a PC. Make sure to download the forms - when you open the files you should see a downward facing arrow icon in the top right corner. This will allow you to fill them in partially and go back to modify later, if needed. Once completed, then choose to “print to pdf” and email the forms to Kim Cima at admissions@stpaulshouston.org.

Important dates:

• The Admission Information, Special Care Needs/IEP and Parent Interview forms are due by May 18.

• The Medical Report is due in July. If you have a doctor’s appointment after this date, please let us know.
